* I do not offer direct billing at this time. This is a paperless clinic and invoices will be emailed after the treatment. I accept debit, credit and e-transfer.

A valid credit card (or visa debit card) is required to secure the appointment you've committed to. I will also be asking for CC info if you call to book. Please have your card info ready.
The card WILL NOT be charged unless there is a violation of the cancellation policy. (CC info is stored securely with a PCI compliant processing company).

If you do not show up for your scheduled appointment, or you haven’t notified me at least 24 hours in advance, you will be charged a missed appointment fee (equal to the full cost of the treatment booked). The fee will be waived if you can find someone to come in your place. If this occurs, please inform me so I can schedule them in and send any necessary forms they may need to fill out.

You must create a profile using their name and information to ensure an accurate health history form. If not done a new profile and intake form will need to be completed upon arrival of treatment and may cut into the appointment time.
